Golf Stretch Academy Liability Waiver & Release​

I acknowledge that I have chosen to receive stretching services from an independent contractor or employee with Golf Stretch Academy, LLC (“GSA”), which shall include all of its owners, members, managers, officers, principals, independent contractors, employees, affiliates, and representatives. I understand that these services may be provided in connection with a golf tournament, special event, demo day, or similar promotional activity.

I have observed and am aware of the nature and possible intensity of the stretching programs offered by GSA. I recognize and understand that, while unlikely, my participation may result in physical harm, which could require medical attention or hospitalization, including but not limited to muscular, skeletal, or nerve damage.

I understand that the services provided by GSA are non-medical in nature, are not a substitute for medical care, and do not include the diagnosis, treatment, or prevention of any medical condition. ​I further acknowledge that assisted stretching involves physical contact and voluntarily consent to such contact as part of my participation.

I willingly and voluntarily assume full responsibility for any and all risks, known or unknown, that I am exposing myself to as a result of receiving services from GSA. By assuming these risks, I understand that I am waiving claims I may otherwise have had.

Health Risks: I warrant that I know of no medical condition that would increase my risk of injury as a result of receiving stretching services. I further warrant that I have disclosed any relevant medical information that could affect my ability to safely participate.

Release & Covenant Not to Sue: In full consideration of being permitted to participate, I hereby voluntarily waive, release, discharge, and agree to hold harmless and covenant not to sue Golf Stretch Academy, LLC, as well as the hosting golf course, country club, tournament organizers, sponsors, event partners, and their respective owners, officers, directors, employees, agents, and affiliates (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, or liabilities of any kind, whether known or unknown, arising out of or related to my participation, including those resulting from the negligent acts or omissions of the Released Parties.

I fully understand that by signing this document, I am giving up my right to bring a lawsuit or make a claim for injuries or damages, even if caused by negligence.

Media Release: I understand and agree that I may be photographed, filmed, or recorded during my participation. I grant the Released Parties the irrevocable right to use such images or recordings for marketing, promotional, or educational purposes without compensation.

Duration: This waiver shall remain in effect for all current and future participation in GSA services unless revoked in writing.

Governing Law: This Agreement shall be governed by and interpreted in accordance with the laws of the State of California, and any legal action shall be brought exclusively in the State of California.

I have read this document in its entirety, understand its terms, and voluntarily sign it, understanding that I am waiving substantial legal rights. This Agreement shall be binding upon me and my heirs, executors, assigns, and legal representatives. If any portion of this Agreement is found unenforceable, the remainder shall remain in full force and effect.
